Address Proof Affidavit

One and Same Person Affidavit
Declares that two differently written versions of your name across documents refer to the same individual.
Change of Signature Affidavit
Declares an updated signature that differs from earlier records, e.g., with a bank or government authority.
Sale / Purchase of Vehicle Affidavit
Declares the sale or purchase of a motor vehicle, commonly required to support RTO ownership transfer.
HUF Affidavit
Declares the composition of a Hindu Undivided Family (HUF) and the deponent's status as Karta, for banking, tax, or property purposes.
Provisional Certificate / Non-Receipt of Original Marksheet Affidavit
Declares that only the provisional certificate or an online copy of the marksheet is held, and the original has not yet been issued/received from the university or college.
No Criminal Case / Clean Record Affidavit
Declares that no criminal case, FIR, or conviction is registered against the deponent at any police station.
Affidavit for Graduation from Another State
Declares that the deponent's graduation degree was validly obtained from a recognized institution located in another state.
Affidavit for Post-Graduate / LLB Degree from Another State
Declares that the deponent's post-graduate or LLB degree was validly obtained from a recognized institution located in another state.
How to Print & Use Your Affidavit
- Buy a non-judicial stamp paper of the denomination applicable in your state (see the stamp duty reference below) from a licensed stamp vendor or your state's e-Stamping portal.
- Print page 1 of the downloaded PDF directly onto that stamp paper. In your print dialog, choose Actual Size / 100% (not "Fit to Page") and set a custom paper size of 21.7 × 30 cm if your printer supports it, so the typed text lands in the blank area below the stamp paper's printed design.
- Print page 2 onward on plain A4 paper.
- Sign in the marked areas, then take the printed set to a Notary Public or Executive Magistrate for notarization.
